Steps:
1. Go to formed.org.
2. Click on "FORMED FOR PARISHIONER."
3. Click on "SIGN UP."
4. Enter your name and email address.
5. Type in zip code 57006 in the field entitled... "Find your Parish or Organization."
6. Select "St. Thomas More" as your parish
7. Click "next."
